The 9 words of Friedberg's Triad have kept >6,000 office-based patients over 25 years from needing hospitalization for either PONV or postoperative pain. Notably missing from ERAS literature is 1) the value of real time BIS/EMG monitoring to minimize over medication and postoperative brain fog; 2) 50mg IV ketamine 3 minutes prior to incision to block midbrain NMDA receptors & eliminating windup phenomenon; 3) avoiding emetogenic opioids. Friedberg's Triad is Measure the brain; Preempt the pain; Emetic drugs abstain. Numerically reproducible paradigm without the added expense of a perioperative surgical home. Food for thought for the open minded.
