A patient presented for free tissue transfer surgery in the upper airway. The plan is for LMA management while a tracheostomy is performed. During surgical marking, pulsations from a high innominate artery are appreciated. These pulsations will be seen via flexible intubation scope placed into the LMA and are used to guide the surgical tracheostomy incision site.
